Each year you should complete Form 8889 and attach it to your Form 1040 if you have HSA contributions or distributions made before April 15, 2010 that is designated for 2009. You will receive Form 5498-SA from your plan custodian, which will give you the amount you (or your employer) contributed during the year. Your employer’s contributions should be shown in box 12 of Form W-2, Wage and Tax Statement, with code “W”. Follow the instructions for form 8889 and complete the Line 5 Limitation Worksheet. Report your HSA deduction on Form 1040, line 28, identified by “Health Savings Account Deduction”. If you received any distributions from the HSA during the year, your custodian will also report this to you and the IRS on Form 1099-SA.
